Hepatitis C Adherence Program

At Integrated Care Systems (ICS), our team of nurses, pharmacists, billing specialists, and support staff understand the challenges facing patients with Hepatitis C (HCV).  We work with the patients and their families to address any educational or psychosocial barriers.  In addition, newer treatment regimens may result in side effects and potential drug-drug interactions.  Our nursing staff educates the patient on side effect mitigation and management, while the pharmacists screen for drug-drug and drug-disease interactions. 

Regular communication with the referring providers ensures continuity of care, as the providers are updated on the patient’s progress, including issues with treatment tolerability, changes in viral load, and adherence.

Our staff are available by phone 24 hours a day, 7 days a week for any questions or concerns from the patients.

Our goal is to improve treatment outcomes at no additional cost to the patient or provider.

ONE-ON-ONE RELATIONSHIPS

Each patient is assigned a dedicated case manager, ensuring a consistent point of contact throughout the course of treatment.

 

PATIENT EDUCATION

Nurses speak with each patient to address any initial concerns, then educate them on the disease state, the need for monitoring, and what to expect from their treatments.

 

DRUG THERAPY REVIEW

Pharmacists screen each patient’s profile for potential drug-drug and drug-disease interactions

 

MEDICATION COUNSELING

Patients are trained on proper injection technique and instructions for oral medications.

 

TEXT MESSAGE MEDICATION REMINDERS

Patients are offered an optional text message reminder service to enhance their adherence

 

SIDE EFFECT MANAGEMENT

Patients are educated on expected side effects and management. Physicians are notified, and prescriptions requested as needed.

 

INSURANCE AUTHORIZATION

Experienced billing staff submit timely authorizations.

 

PATIENT FOLLOW-UP

Patients are contacted regularly to review and explain lab results, assess tolerability, and review management of side effects.

 

SCHEDULING AND MONITORING OF LABS

Nurses remind patients of lab appointments, follow up on missed appointments, and contact physicians regarding out-of-range results.

 

MENTAL HEALTH MONITORING

Patients are regularly monitored for potential depression or mental health issues.
